Invitation to Join a Webinar on Advance Care Planning

Individuals and teams who are actively involved in planning and developing Advance Care Planning (ACP) initiatives in their organizations/regions were invited to attend a webinar on May 7, 2013 on ACP. This information session was hosted by the Canadian Foundation for Healthcare Improvement (CFHI) and organized in partnership with a steering committee of healthcare leaders who are advancing ACP in diverse sectors.

The objectives of the webinar were:

  1. To present an overview of the current ACP landscape in Canada
  2. To highlight examples of innovative tools and partnerships
  3. To consider opportunities for working together to improve ACP in Canada

The following steering committee members participated in the webinar:
