WHAT: A small proportion of patients with complex needs often accounts for as much as two-thirds of health care use and costs. Quality Improvement via Appropriate Prescribing

WHAT: Fifteen healthcare organizations and 54 long term care/skilled nursing facilities have improved the quality of life and health outcomes of their residents through participating in CFHI's recent 16-month collaborative to reduce the inappropriate use of antipsychotic medication. CFHI and team representatives will present innovative strategies used to implement, measure and spread improvements, including improvements in cost efficiency. These approaches can be adapted and applied to facilitate healthcare improvements elsewhere.

Better Patient Safety with Lean in Saskatchewan

WHAT: In 2011, Canada's Saskatchewan health care system suffered from long wait times and an estimated 5,600 annual adverse events, including 495 patient deaths. To improve, the province's 40,000 health care providers began a transformation to lean methodology. Four years later, their case provides crucial insights into the success factors, pitfalls, and impressive results of implementing lean in a huge health system.
