Horizon Health Network offers hope to youth with mental health issues at PEER 126
In Saint John, New Brunswick, there was a pressing need to develop supports for youth living with mental illness, and studies demonstratethat early intervention can prevent a lifetime of struggle. To fill this need, Horizon Health Network founded PEER 126 (Peers Engaged in Education and Recovery), a place where young people experiencing mental health challenges can visit a drop-in centre.

Western Health (Newfoundland) invests in better selfmanagement support for people living with diabetes
Working with the Canadian Foundation for Healthcare Improvement through the AHC, Western Health developed a new approach to Diabetes care with self-management as its cornerstone. Focusing on each patient’s emotional health, patient input and staff training, the new approach incorporated many changes, including types of services offered, how services were delivered, referral mechanisms, and more.

Health PEI supports realistic behaviour change for Islanders living with chronic disease
Working with the Canadian Foundation for Healthcare Improvement (CFHI) through the AHC, Health PEI standardized training sessions in chronic disease self-management to train their primary healthcare workers.
