“Like other healthcare organizations, we face challenges in providing the best possible care to our patients who have complex health issues. The Better Health and Lower Costs Collaborative will enable us to offer patients a coordinated series of community services in partnership with other social and healthcare providers. We believe that this approach will be cost effective and, more importantly, it will help to ensure that our patients have optimal outcomes.

Better Health and Lower Costs for Patients with Complex Needs helps healthcare delivery organizations plan and implement comprehensive care designs that serve the needs of their most complex, high-risk, and costly patients resulting in better health outcomes, a better care experience and lower per capita cost.
Over 12 months, participants:
- Identified a high-risk population group
- Assessed the needs and assets of this population by learning from patients’ experiences
- Co-created and executed new care designs to test for impact and cost savings
- Spread and scaled up successful care designs
