The Canadian Foundation for Healthcare Improvement (CFHI) is once again teaming up with the Institute for Healthcare Improvement (IHI) to improve healthcare in Canada. This time CFHI is supporting 10 Canadian teams to participate in the Better Health and Lower Costs for Patients with Complex Needs Collaborative. Previously CFHI supported nine Canadian teams in the Triple Aim Improvement Community.

CFHI is sponsoring 10 Canadian healthcare delivery organizations (at $10,000 each) to participate in this new IHI 12-month Collaborative to achieve better and more cost-effective care for their most complex, high-risk patients.

Since September 2013, with CFHI’s support, nine Canadian healthcare delivery organizations have been participating in the Triple Aim Improvement Community (TAIC), a 10-month improvement community to achieve better population health, better healthcare experiences and improved per-capita cost of care. The nine Canadian organizations are part of a larger international cohort, which includes organizations from the United States, Denmark, Sweden and the United Kingdom.
