Join us for our afternoon breakout session, Triple AIM: Delivering better health at lower cost, while reshaping culture in Ontario healthcare organizations

Hear why successful healthcare systems of the future will be those that simultaneously deliver excellent quality of care, at optimized costs, while improving the health of their population. This is known as the IHI (Institute for Healthcare Improvement) Triple Aim. In 2013, we collaborated with IHI to support Canadian team enrolment in the “Triple Aim Improvement Community.” 

In this session, participants will:

  • Find out how healthcare delivery organizations in Canada are interpreting and applying the IHI Triple Aim, with specific reference to change packages for “high-risk, high-cost” patient populations
  • Learn how two Ontario healthcare organizations are delivering Triple Aim innovations to drive systemic quality improvement (as it relates to Health Links and delivering Quality-Based Procedures)
  • Discuss what’s required by way of leadership, culture and improvement capability to deliver on the Triple Aim promise

CFHI Innovation Station

Join us for a mixed media and interactive display. This would include a rotating video featuring improvement projects from programs such as the EXTRA program for healthcare improvement and the Patient and Family Engagement Projects. We will feature four storyboards describing innovative CFHI-supported projects that have led to measurable improvements in quality, accessibility, safety and efficiency of healthcare services in Canada. Throughout the display, our stories will describe the innovation as well as the change management process and the evaluation of results. On display, will be a whiteboard on which you can write down what you think are the most promising innovations in healthcare delivery.
