Teams sustain and spread quality improvement efforts

Many teams that have recently completed CFHI programs are participating in the Momentum Challenge, designed to help teams grow the gains they have already made to spread and scale their innovative quality improvement initiatives – and achieve long-term success.

Teams from the INSPIRED COPD Scale, Connected Medicine and New Brunswick Appropriate Use of Antipsychotics collaboratives were invited to join the first cohort of the Momentum Challenge.

In addition, teams from the Newfoundland & Labrador and PEI Appropriate Use of Antipsychotics and Embedding Palliative Approaches to Care collaboratives were invited to join the second cohort of the Momentum Challenge.

More than $480,000 in awards in total will recognize measurable impact and reward success in moving the dial on key indicators and increasing the number of patients reached.

As of November 2019, the first Momentum cohort includes 15 participating teams: 93% of teams who submitted data sustained their quality improvement initiative and 60% spread it further. The second Momentum cohort is going strong with 50 teams .

The first Momentum cohort will be complete in April 2020 and the second cohort will be complete in October 2020.
