Improving Provider-to-Provider Communication

Timing: May 2016 to January 2017

Ten improvement teams from across Canada and internationally participated in an 9-month Access to Specialist Consult e-Collaborative. The initiative aimed to support improvement teams in enhancing primary healthcare access to specialist consult (provider-to-provider) services through the use of electronic and/or telephone consult.

The Access to Specialist Consult e-Collaborative also supported the Patient’s Medical Home (PMH) model, a vision for the future of family practice in Canada spearheaded by the College of Family Physicians of Canada (CFPC).

Evidence-based learning

Participating teams learned from two Canadian evidence-based and patient-centred specialist consult models:

  1. Rapid Access to Consultative Expertise (RACE) – a telephone advice line launched at Providence Health Care / Vancouver Coastal Health in 2010; and
  2. Champlain BASE eConsult Service (BASE) – a secure web-based eConsult service launched within Champlain Local Health Integration Network in 2010.
