Strategies for Population Management: An IHI Triple Aim Workshop hosted by CFHI and Health Quality Ontario

This workshop attracted participants from eight provinces – BC, AB, SK, QC, ON, NS, PEI and NL. Offered for the first time in Canada, it combined principles of population health management with the Institute for Healthcare Improvement (IHI)’s Triple Aim framework. Faculty from IHI and other U.S. and Canadian organizations provided participants with the knowledge and skills they need to successfully pursue the Triple Aim in their organization or community.

What is the Triple Aim?

The Triple Aim framework, developed by IHI, refers to the simultaneous pursuit of better care for individuals, better health for populations, and lower per capita costs and has been piloted with over 100 organizations around the world. Population health management strategies will help you keep your patient population as healthy as possible, minimizing the need for expensive emergency room visits, hospitalizations, and other health services.

What did participants learn?

  • How to apply effective strategies for integrating the Triple Aim in your organization.
  • How to draft a portfolio of projects tailored to the population they serve.
  • How to implement population-specific change ideas to improve all three Triple Aim dimensions: care, health, and cost.
  • How to engage and activate community partners as full participants and leaders.
  • Lessons and tips from senior leaders and improvement teams that have successfully applied Triple Aim in their organizations.

What can participants do with the tools and skills acquired in the workshop?

  • Identify a Triple Aim population and strategic sub-populations.
  • Develop or identify a leadership or governance structure to champion the work.
  • Craft a compelling purpose statement.
  • Develop a portfolio of projects which, taken together, move the organization or community toward the Triple Aim.
