Canadian Thoracic Society COPD Action Plan

2013

The Canadian Thoracic Society Action Plan is a tool that aims to help those diagnosed with COPD prevent and manage their exacerbations by working as partners with their healthcare team. It uses a ‘zone’ system to help healthcare providers, patients and caregivers recognize when further support and treatment is needed (e.g. green zone deals with baseline and wellness symptoms while the red zone indicates a need to seek emergency medical treatment). The goal is early and appropriate intervention in order to minimize the impacts of COPD exacerbations on patients and caregivers.

You can download the Action Plan via the Family Physician Airways Group of Canada website.
