COPD is the most common cause of admission into acute care facilities in Canada. When the burden of increasing symptoms limits patients to their homes, accessing appropriate care becomes increasingly difficult.
This article outlines the need for clinical interventions for COPD and reorganized models of care designed for better continuity. It argues for moving toward an integrated approach to COPD management, including advance care planning, which will help patients and caregivers with medical decision-making.
