INSPIRED COPD Outreach Program™ Patient Consent Form

This form is a template consent form that is used by the INSPIRED COPD Outreach Program™ in Halifax to recruit patients to participate in the program. Patients are asked to give their consent for details, including records of their visits/admissions to hospital and COPD-specific information, to be provided to the program team. The information is used to monitor the clinical care patients receive.
