Providing patient and family-centered care and improving patients’ care are important health system goals. Engaging patients at provider, micro-system and organizational levels is key to achieving system-wide improvements in these goals. This is illustrated in the three Levels of Patient Engagement model shows.
The three levels involve engaging patients in shared decision-making and self-management, and in program planning and quality improvement, and developing and spreading a culture of engagement by weaving patients and families into all activities of the organization.
