Foundations of Quality Improvement for Long-Term Care, Assisted Living and Retirement Homes

This guide supports long-term care, assisted living and retirement home providers, team leaders and staff to partner with residents, families and care partners to successfully apply quality improvement (QI) techniques. Through a series of short steps, you can learn to initiate, test, implement and spread evidence-based best practices.

From QI foundations, applying the improvement process and compiling measures, to implementing then planning for sustainability, working through these steps will help all those involved to introduce and spread successful, sustainable change.

The guide was developed as part of the LTC+: Acting on Pandemic Learning Together (LTC+) program being led by Healthcare Excellence Canada (the new organization that brings together the Canadian Foundation for Healthcare Improvement, the Canadian Patient Safety Institute). BC Patient Safety & Quality Council and others. The LTC+ Coaches use this tool when working with the LTC+ teams.
