EXTRA by the numbers

To make change happen, we need strong and capable leaders – those who have the knowledge, skills and experience to co-design innovative approaches to address complex healthcare and social services problems. Leadership capacity to support and lead change is an essential ingredient to accelerating the spread of evidence-informed improvement.

Launched in 2004, the Canadian Foundation of Healthcare Improvement’s EXTRA program is a recognized team-based leadership development and quality improvement fellowship. It is the only one of its kind in Canada. The program is grounded in leading and managing change, and sustaining gains, in the reality of today’s complex healthcare and social services environments.

Through the EXTRA program, CFHI has supported more than 156 organizations, 455 leaders in healthcare and 243 improvement projects. Some of CFHI’s programs originated from EXTRA teams’ improvement projects, including the Appropriate Use of Antipsychotics and Connected Medicine spread collaboratives.

How EXTRA contributes to better healthcare in Canada

The EXTRA program is more than an investment in individuals. It builds leadership and organizational capacity for participating teams, and their organizations, to achieve evidence-informed improvement that enhances care and experiences for patients, families, caregivers and healthcare providers. EXTRA can also improve value-for-money at provincial/territorial and national levels.

Through EXTRA, fellows:

  • Improve key leadership skills required to lead change and large-scale improvement
  • Have access to a network of renowned Canadian, and international experts and leaders
  • Receive integrated, hands-on coaching and guidance from expert CFHI faculty and coaches
  • Learn through a combination of self-reflection, teamwork, workshops and the improvement project
  • Have the opportunity for pan-Canadian peer-to-peer learning and networking.

Organizations benefit from EXTRA by having:

  • Leadership skills among senior staff, with the expertise to transform care
  • Leaders with skills to convert evidence into actionable policies and programs
  • Increased capacity and readiness to implement ongoing sustainable improvements
  • Ways to create a culture of learning by fellows’ sharing their learning within their organizations and across the health system

Each team receives support to effectively evaluate their project, with the goal that their healthcare improvements will be spread and sustained over the long term, and continue to benefit more people.

How EXTRA works

The 16-month EXTRA program draws on CFHI’s Six Levers for Healthcare Improvement, and designed to build leadership and organizational capacity to transform the healthcare system.

Teams apply to join EXTRA, and carry out improvement projects that address real-life health system challenges by finding and implementing evidence-informed solutions. There are usually three to four fellows in each team, all who have significant senior responsibility and influence in their organization. The teams can involve multiple organizations including: health and social service providers, health authorities, ministries, administrators, and patients or other people with lived experience.

Teams receive dedicated support and coaching for the design, implementation and evaluation of their improvement project. Fellows complete leadership development modules and attend in-person workshops and site visits, have regular coaching calls, take part in interactive webinars, and have access to an online learning platform containing tools and resources. CFHI subsidizes most of the financial costs for participating teams.

As a CEO of a participating organization, I can tell you that EXTRA has been a great investment. It has already made changes in the way my senior leadership team approaches projects and how we work together with patients and families. With EXTRA you have a better quality program, better quality training and I can’t see an equivalent anywhere. It’s not that I haven’t looked, we’re always looking to improve our knowledge.
